Ordering additional Death Certificates

Death Certificates may be ordered from county health departments throughout the state.  Consult the web site of the counties on the map below for ordering instructions. Orders may be placed in person, or by mail, fax, and online.

Some counties hold death certificates for only 60 days from the death of the individual and are thereafter forwarded to the State Center for Health Statistics for filing and archiving. You can reach the State Center of Health Statistics by calling (360) 236-4300.
